Has anyone else had problems with the new 613 brake cable stops popping off of the top tube, or heard of the problem?

Yes, Cannondale actually sent out a tech. update stating that the rivets (that hold the brake cable to the tube) were not long enough. Take it back to the shop for a warrantied fix - they'll replace the rivets. If they don't, claim the warranty and get a new frame.
