Help! New shoes/same cleats--right lateral foot pain

Just got a pair of Sidi Zeta's and am using Crank Bros Quattro pedals. Old shoe was a pair of Pearl Izumi Vortex(? I think). Both shoes fit well, and I've used the Quattro pedals since Aug 2006. Even after tweaking the right cleat multiple times, the lateral side of my right foot still hurts after rides. The longer the ride, the more the pain. It almost felt like a very mildly sprained ankle the 1st time out.

What gives? Velcro too tight? Shoe too narrow? Anything else I'm doing wrong in cleat adjustments? The area in question is labeled #12 in the picture.

Thanks in advance for your help!

Cleat position? Try moving it inwards/outwards and see how it feels.
